II. Proposal for Teaching & Learning Grants

Include the following:

a. Overall goal of the project
b. Give a brief, specific description of the need for the proposed professional development
c. Grade-level focus
d. Mathematics content
e. Describe how the proposed project will be conducted and how it will meet the identified needs listed above (in part b.)
f. Timeline
g. Number of teachers impacted
h. Number of students impacted
i. Mathematics education consultants, coaches or advisors involved in the project and describe their role.

III. Budget Request Form

Include an itemized budget listing funds requested from ICTM, district financial support, and other sources of funding.

V. Principal’s Letter of Support

a. Letter must be signed on school district stationery and indicate strong support for the proposal.
b. Any district assistance should be stated in the letter. This support would include district financial support, materials provided or purchased, and teacher release time.

VI. Final Report Requirements

Teaching & Learning Grant awardees will write a short blog post (300-500 words) for the ICTM website or submit three to five 1-minute videos detailing the activities and learning outcomes of the project. If materials were purchased with the grant, they should be included in the videos or pictures submitted with the blog post. These will be used on the ICTM website and/or social media platforms.

ICTM Conference Grant awardees will submit the receipts for registration and other travel costs to receive reimbursement. Additionally, they will write a short blog post (300-500 words) for the ICTM website or submit three to five 1-minute videos documenting my learning from the conference experience. These will be used on the ICTM website and/or social media platforms.

NCTM Conference Grant awardees will submit a receipt for early bird registration, which will be reimbursed upon receipt. After the conference, they will submit the other receipts for travel, hotel, food or sub costs to receive reimbursement. Additionally, they will write a short blog post (300-500 words) for the ICTM website or submit three to five 1-minute videos documenting my learning from the conference experience. These will be used on the ICTM website and/or social media platforms.

VII. Reimbursement

Reimbursement of expenses will be paid in a timely fashion upon the grant awardee

• submitting an itemized report of expenses consistent with the original budget request,
• submitting receipts for expenses,
• completing one of the options for sharing with Iowa’s mathematics education community.

Reimbursement will be sent to the primary grant applicant or to a designated recipient. Variation from the items in the budget request must be approved prior to expenditure and subsequent reimbursement.
